The only problem was, as I discussed in my previous blog post, my face. A mysterious bacterial infection had developed in my beard area—as if to punish me for growing an actual beard for the first time in my life—and seemingly set up shop. Despite a course of doxycycline and topical clindamycin lotion, the first-line treatments for impetigo and other superficial skin infections, my face went from bad to worse to full-on freak-out.
It should come as no surprise, in retrospect, that the bacteria that had colonized my face had a mind of their own. Bacteria were among the first life forms on Earth, so it stands to reason they know a thing or two about survival.
I could only watch in dread as my face morphed from day to day in a feeble attempt to mount a response to this new invader. It was at this point, in mid-August, that the follicular nature of my skin condition became more evident. Whereas before, in late July, it seemed to be a broad, generalized infection of the superficial skin classically seen in impetigo , it now appeared to be originating in the hair follicles themselves.
I still had significant swelling in both cheeks, but I was now developing painful cystic lesions, usually a pustule by definition, a pus-filled vesicle with a centrally placed hair. All I knew was that these beard hairs hurt like tiny, excruciating needles. The only relief I could get was from pulling the offending hair out with a pair of tweezers. Usually the bulb of the hair would be covered with a greasy coating, and from the now empty pore a substantial amount of oily, yellow fluid would inevitably ooze.
I also attempted to shave a few times in these early months, reasoning that topical antibiotics would be ineffective if I continued to grow a beard. This also was a mistake, most likely redistributing the bacteria to previously unaffected follicles—like a bee pollinating flowers. In seeking temporary relief from the painful cystic lesions, I unwittingly delayed long-term control.

This month, husband-wife dermatologists Bill Danby and Lynne Margesson share their patient handout on isotretinoin, which Dr. Danby of the drug long known to the public mainly by the original brand name Accutane, which is no longer available.
Yet his handout, which is featured this month, makes it abundantly clear to his patients that there is a price to pay for the freedom it offers from nodular acne and the scarring it causes. Less commonly the dryness extends to eyes and eyelids, leading to blepharoconjunctivitis or problems with vision. For all, Drs. Danby and Margesson offer tips including various emollients, sunscreens, eyedrops and vitamins.
Other less common side effects, they warn, require other types of treatments. The rarest side effects include pyogenic granuloma, dissemination of existing herpes lesions, skin fragility or skin infections such as impetigo.
And in the rare instance of blood in urine, it is usually due to dry split skin in the area. General Side Effects The handout also covers a long laundry list of other side effects that have been reported, including headaches, depression, arthralgia, myalgia, blood sugar abnormalities, tinnitus, and gastrointestinal problems.
Patients are made aware that various blood tests will be altered by isotretinoin and that half experience an elevation in blood fats. The Side Effect That Must Be Prevented… But, as all dermatologists know, for women there is a far more worrisome side effect — birth defects — the prevention of which requires vigilant ascertainment that patients are using at least two forms of effective birth control.
Their negative pregnancy status must be verified not only before a first prescription, but every month before they can receive a new one. There is no misunderstanding the anti-pregnancy message in their handout: Any pregnancy occurring on this medication must be terminated.
Your acceptance of a prescription for isotretinoin means you accept these facts, you will report any pregnancy to your dermatologist, and you will comply with termination an abortion in the event of any pregnancy.
